No — you do not need a doctor’s referral to see us. Nebraska allows Direct Access to physical therapy, which means you can book an evaluation and start treatment right away without needing a script from your physician.
If we ever believe your condition requires imaging or further medical evaluation, we’ll refer you to the right provider — but in most cases, you can start the process immediately.

Yes — we accept HSA and FSA cards for all services at Forward. Because we’re a licensed healthcare provider, most clients can use their Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay for physical therapy, performance testing, and evaluations.
We’ll provide detailed receipts and documentation if your HSA provider requests verification.
Your first session is all about clarity and direction — understanding what’s going on and building a plan to fix it.
We start with a deep dive assessment of your movement, strength, and mobility — not just where you feel pain, but how your whole body moves. You’ll get a clear explanation of what’s driving your issue and why it hasn’t improved yet.
From there, we’ll create a custom rehab and performance plan that’s tailored to your goals — whether that’s running pain-free, lifting heavier, or getting back to your sport.
By the end of your evaluation, you’ll walk out with:
• A clear diagnosis of movement limitations and strength deficits
• A step-by-step plan specific to you and your goals
• Specific exercises so you can immediately start seeing a path Forward and experiencing relief
You’ll leave knowing exactly what’s happening, what to do next, and how we’ll help you move forward.
Wear comfortable, athletic clothing that allows you to move freely — think what you’d wear to the gym or a workout.
We’ll be assessing how you move, squat, run, and perform different exercises, so shorts and a t-shirt or tank usually work best. Running shoes or training shoes are ideal, depending on your goals.
If we’re doing a Runner Analysis or Performance Test, bring the shoes you typically train or race in — plus any insoles or gear you use regularly. Bottom line: if you can move in it, it works.
Every session at Forward is one full hour, 1-on-1 with your Doctor of Physical Therapy — no techs, no overlapping appointments.
That gives us time to assess, treat, and train in the same visit — addressing pain, mobility, and performance together instead of rushing through a checklist.
Your initial evaluation is also 60 minutes and includes a full-body assessment, goal-setting, and your first custom program in our app.
If you’re doing Performance Testing (VO₂ or lactate), plan for about 60–75 minutes depending on your test and setup.

Once you’ve finished physical therapy, most clients transition into our Performance Plans or Hybrid Personal Training. These programs build on the foundation we created in PT — focusing on strength, running mechanics, mobility, and performance goals specific to you.
You’ll stay with the same Forward team, using the same app and tracking system, so there’s no drop-off or starting over. Think of it as Phase 2 of your progress — going from pain-free to performing better than ever.
